The unnamed woman unmasked.
It appeared while the gang were in a hallway, scaring them away before flying away. It later appeared near the end, being unmasked, along with a headless waiter and a frog-like monster, revealing an unnamed woman who was wearing the costume.
Due to the fact it's appearances are exclusive to the show's opening sequence, the unnamed woman's motives or reasons for dressing up as it are unclear.
